Our recent collaboration with “A Nurse in the Family” is a perfect example of how teamwork can make a meaningful difference in a client’s life. By working together to successfully complete a home safety project for a mutual client, we were able to combine clinical insight with practical home modification solutions—ensuring the client’s environment is not only safer, but truly supportive of their independence and well-being. This project highlights the value of our relationship and the powerful impact of coordinated care in the community.

“My client is a resident of Rossmoor and their daughter—an only child living clear across the country—was the one who finally reached out. There are so many people out there aging alone, with an ill spouse, and trying to navigate solutions they don’t even know exist. They believe that the hospital will send out help when really what people get is cookie cutter services that do not help- it just delays finding the right help. The daughter called because her father wouldn’t stop falling and her mother needed much needed support.  Her father’s falls were relentless—almost every day brought a new injury. For seniors, these falls typically end in broken hips or head trauma. He’d been lucky so far. The family had been lucky, too, in finding A Nurse in the Family.   

Within one month, everything shifted.

We established comprehensive support: physical therapy tailored to his specific needs, a home health company to give his spouse some much-needed rest while also providing in-home assistance with bathing, dressing, and light meal preparation. Home Safety Services responded within the same week—they knew this was time-sensitive. One more serious fall and he might not be able to live at home anymore. Or worse. 

We reviewed his medications and discovered they could be contributing to the falls. We modified doses while arranging a doctor’s appointment. We secured essential equipment—a walker, proper shoes, everything he needed to help stay safe.

The daughter and wife could finally breathe. The goal is to support people where they want to age with the level of support they need. Sometimes, all it takes is knowing where to start.”
